After over a decade of practice in the Functional Medicine model it has become apparent to us that the best outcomes are achieved when patients have regular access to Dr. Jacobs as the relationship is being established and the initial treatment plan is developed and tweaked. To better reflect this model of care our new patients are enrolled in comprehensive care plans for the first 2-6 months of care depending upon initial needs. Following this initial period, patients can choose to continue in a comprehensive care model or to pay for care on a visit-by-visit basis based upon their progress and ongoing needs.

Well Woman Care $1,200/2 months
This is for women who are in good health and want a Functional Medicine approach to risk assessment and a personalized plan for optimal wellness. Comprehensive intake, risk factor assessment, recommendation for labs testing, lab assessment, follow-up appointment as necessary, portal access, telehealth/telephone consultations.
Dr. Jacobs is not contracted with any insurers. Coded statements will be provided; those with out-of-network coverage can submit for reimbursement.
